When I'm in the pager and on the first message, if I accidentally press
it takes me to the index, with a message of "You are on the first
message". This makes sense, but I would prefer to stay in the pager.

I don't know if it's an option for your situation, but you might
consider offlineimap. For a folder of size about 1000, it took 2 seconds
to open it. Of course, this is after using offlineimap to download the
messages locally.
