Most any frame builder who is comfortable also making stems can make you that stem. Off the top of my head I'd ask Mitch Pryor, Brian Chapman or Eric Estlund of Winter Cycles. Both routinely make very nice quill stems for their bikes, but I'm not sure if they'd do one-offs for another bike.

Keep in mind that when you're riding the tension on the chain will make it a little more slack. I adjust mine to be a tiny bit tighter than I want when it's in the work stand. While pedaling, the chain will be properly tensioned. Also, single speed setups aren't as sensitive to chain tension
