We just received Disease Specific certification from the Joint Commission. One 
are that we need to work on (OFI) was with patient satisfaction survey. Does

Hey Alexis, we use the St. John's Sepsis Alert here at the University of 
Tennessee Medical Center.

We require the nurse to contact their Clinical Nurse Specialist or Clinical 
Nurse Educator immediately when an alert has fired. The nurse and specialist 
will decide whether or not to notify a provider.  (our policy is to always 
notify a provider unless an erroneous value was accidentally entered or if the 
patient should have had a "snooze" for the alert). We do not allow any actions 
to be done just from an alert being fired, but we are a large teaching 
facility: our "alert-to-orders" times are short because of easy accessibility 
of providers.

I am required to track all alerts and actions performed due to the alert 
(highly recommend this!).

Yes, we use Cerner and the Sepsis alert.  The expectation is that the nurses 
notify the doctor within 30 minutes of receiving the alert 100% of the time.  
One thing to watch for is that the alert links to a generic clinician notify 
form, so the nurse needs to document why the physician was notified in the 
comments field ("Sepsis alert received" is sufficient) otherwise it reads that 
the doctor was notified but we don't if it was for pain meds, or lab results, 
or something else.

Hi,

We have used Cerner since 2013 and have edited the algorithm to meet our needs. 
 The process is a discern alert for nursing that messages the information 
needed to call the provider within 30 minutes.  The RN documents that call in a 
critical value form that fires from the alert.  We have an auto-order for vital 
signs and lactates also.  The providers get an alert up to 24 hours after when 
they open a chart or write orders.  Our providers don't have the correct 
relationship with the patient in order to fire directly to them in a timely 
manner.
If the patient meets criteria for septic shock (criteria directly on the 
alert), the process is to call 911 Sepsis Code and the RRT responds.
