Finally! The solution for #2 is to do:
partman/alignment=cylinder
After this bootparam (in pxelinux.cfg/default), mdadm did the right
thing and the system booted just fine.
Note that this is done on 2 different x86_64 systems:
1. Sun workstation Ultra20 m2
2. HP xw4600
Both have 2 SATA disks

I have the same problem... I need to press the eject button on my CD
drive three times, First when it is closing again... to get it out, when
it pulls in, press eject, the tray goes out an then in again, pressing
eject one more time here let the tray stay out for me.
