It happens to me too on a freshly installed Oneiric (AMD64). The problem manifests itself quite consistently when I open terminal, but yesterday it happened a couple of times with Chrome too. I'm sure sometimes it doesn't do it because I was able to select Keep in launcher for the terminal. When it was on the launcher, I could run the terminal from there, but the small triangle wouldn't show up and clicking on the icon wouldn't give focus to the application. And terminal doesn't show up in the ALT-TAB selector either (see attached screenshot). As I said above I ran SeaTools for Windows' Long Generic Test and it didn't report any problems (you can find the log file in my previous post) while palimpsest reports 466 bad sectors (see attached screenshot). I don't know if this is a credible number of relocated sectors. I'm willing to run more tests, just tell me what to do. Same here on my fresh Karmic installation (x86_64), using a Seagate Barracuda ST31500341AS. I also tried the 32 bit version of Karmic and got the the same results. I re-installed the system, so now I have a fresh installation of Ubuntu 9.04. I tried connecting the laptop to a different monitor (Samsung SyncMaster 226BW), nothing changed, so I guess it doesn't depend on the VGA/monitor combination (not that I really believed that the monitor had anything to do with it, not I'm sure it doesn't). I had successfully configured my old monitor (a 19'' Dell LCD) for its optimal resolution (1280x1024). There appears to be nothing wrong with the hardware since everything works fine in Windows (I had to manually add the resolution there too though). The problem seems to be that nvidia-settings is unable to detect all the resolutions the monitor is capable of displaying, the maximum I can get is 1280x1024. I also tried manually adding the resolution to xorg.conf but when I restart X I get no video signal (neither on the external monitor nor on the laptop display). I had this problem with Intrepid and also with the new jaunty beta (I upgraded just to see if something had changed).
