I believe you double tap and hold on the image, after which you will get
prompts, one of which I believe is save; it will go to photos. If the double
tap and hold doesn't do it, if you double tap on the attachments, you may get
options that include save. It has been a while, so is why I am a

Can you explain a bit more what you are trying to do? I am not sure what
picture you are trying to save, e.g. where is the picture and to where are
you trying to save it?

I haven't used this feature, but apparently there is a bug in iOS 6 which
doesn't allow you to do this with Voiceover and the guy who does the Podcast
