Caroline, that sound is supposed to play and while there seems to be some inconsistency with the choices, the Do Nothing works just fine and will get rid of it. I would be curious to know what the other reasons are why you find iOS 15 is a mess. I actually think it's one of the least buggy iOS

The fact remains that it seems the choices in that section don't necessarily do what they claim to do. I don't use Braille so while I think if Speak and Braille are selected, the "Actions Available" message is spoken, if only speak is selected it is not and if only Play Sound is selected the

Hello Richard, If you look at Verbosity under Accessibility, VoiceOver and Verbosity there is an Actions category. Under this category you have the option to Play a Sound. If selected then the sound you have recorded will play. I have mine set to: Actions, Speak, Braille. Hope this helps.
