I had a tensioner start to go on my old 93 4x4 KA24E.  The chains and 
tensioner were shitty in those years because they insisted on using a single 
chain, not a double, so they stretched over time at around 90,000 miles.  
Also, Nissan used a plastic guide for the chain which wore out if you didn't 
keep the oil really clean.  I noticed mine going bad at 100,000.  During a 
warm startup you could here this grinding of the chain slapping off the 
cover!
