[Sepsis Coordinator Job Description]
* Full time FTE
* In conjunction with sepsis physician champion, leads quality improvement
work in an attempt to build a robust process that relies on evidence and is
able to deliver a reliable, auditable, and sustainable culture of care for
septic patients throughout the organization
* Educates, engages, and attempts to move metrics with the goal of 24/7
sustainability with frontline practitioners and nurses - Process should ideally
work independently without the need for micromanagement
* Process owner for all sepsis protocols, policies, tools, and
powerplans/ordersets - in charge or roll-out and go-live of all implementations
and updates related to these
* Facilitates and heads up monthly Sepsis Charter/Steering Committee
meetings, Septic Shock M&M meetings, and Regional Sepsis Conference calls
* Heads up daily severe sepsis and septic shock surveillance on all
inpatients
* Completes data entry into database on all severe sepsis and septic shock
patients outside of P2/ICU which are entered by Gail Manley - ICU Apache

* Reports to Anne Cantrell - Interim Manager of Performance Improvement &
Diane Barton - Interim Director of Performance Improvement and Patient Safety
