The report doesn't say. Keep in mind that the report also does not actually support a claim of silver toxicity. Some excerpts from the report:
"This case demonstrates the risk of diets that are lacking in protein, calories, vitamin B 12 , iron, and thiamine. The potential consequences of such a diet in infancy have been described widely and include death, permanent neurologic impairment, and developmental delay (1-4)." "It does, however, have potential toxicity. Silver accumulates widely in the body and can cause a diverse range of problems, including hepatic necrosis, gastrointestinal hemorrhage, permanent skin discoloration, neurologic deficits, and glomerular damage (8). Despite the high levels documented in our case, we could find no evidence on clinical examination or biochemistry of such effects having occurred, apart from the child's developmental delay, which could be explained by the other nutritional deficiencies." What I take from this is NEVER tell a physician you are using CS.
