You've got 4 DZV11's in the config.  Did you actually add all of them to the 
SYSGEN CONFIG?

Actually, the boot ROM's display of SHOW DEV that sees DUB at 0760374.  This is 
only due to the fact that it is at the expected address.  The table which the 
simulator auto config logic uses is the same table the boot ROM and VMS uses to 
determine the appropriate addresses.
