Does it only have significance to the entity that receives it?

Yes, I have seen this.  For example:
        tel:1234;phone-context=+1-555-987
        is the same as:
        tel:+1-555-987-1234

But does a context such as the following have any meaning?
     sip:1234;[EMAIL PROTECTED]

How is the freescale.com part resolved?
Is there a global registry of these contexts?
Or is it  something that only means something to 
the device that issued the URI with that context?

This is incorrect. phone-context is part of tel url and is not sip
domain name.

I do not think so. "phone-context" is applicable for local tel numbers
and indicates which context the number belongs to. For example, the tel
number can represent a local number where the phone-context will
indicate combination of country and area code.
