Maybe I don't fully understand your request here, but the E.164 format relates to calls from "outside". One would not give an E.164 number to internal calls, rather you would assign a DID to this to achieve the same result.
We use bandwidth.com with an ingate and our dial plan strips the "+" on incoming calls. None of the aliases have a "+" in them. Depending on your phones and firmware, we also have dial plans that strip the "+" on outgoing calls when dialing from the call list in the phone and send them only digits.
