this should work: 5digit extensions, lets say yours is 85060

you want caller id out to be 404-778-5060

leave caller id blank in user (and group)

in gateway, (sip gateway as an example, if you have an analog, it might be the problem) and I think I have used 10 digit users without any problems.
in gateway, under caller id, advanced settings:

leave default caller id blank.
select 'transform extension', in caller id prefix, put in 40477
keep digits, 0 or 5 should be ok.

(ps, it takes about 2 mins for this to be active. call your cell phone to test :-)
