I was pretty sure I had tried that exact config, but for good measure I tried 
it again.
Same result.  Only 5 digits are sent.
The extension is 12099.  In the gateway, I selected "Transform Extension", 
added caller ID prefix 40425, and tried first with 0 (keep all digits), and 
then with 5 specified.  I even tried using this page with fewer prefix digits, 
but it's as though they are ignored.

It still sent only 5 digits.

this should work: 5digit extensions, lets say yours is 85060

you want caller id out to be 404-778-5060

leave caller id blank in user (and group)

in gateway, (sip gateway as an example, if you have an analog, it might be the 
problem) and I think I have used 10 digit users without any problems.
in gateway, under caller id, advanced settings:

leave default caller id blank.
select 'transform extension', in caller id prefix, put in 40477
keep digits, 0 or 5 should be ok.

(ps, it takes about 2 mins for this to be active.  call your cell phone to test 
:-)
