The steps I use are:

Make sure you get the current Repo files and place them in
\etc\yum.repos.d.  If you are running on CentOS:
cd /etc/yum.repos.d
mv sipxecs-centos.repo sipxecs-centos.repo.old
wget http://download.sipfoundry.org/pub/sipXecs/sipxecs-centos.repo

Check DNS to see if you are going to trash any custom DNS entries.  If there
are custom entries in your zone files (located in /var/named) you may want
to backup the zone file and/or set the zone files to manual in named.conf
(change the line that says 'Master' to 'Manual').  To edit the named.conf
file:
nano -w /etc/named.conf

Backup the system from the GUI.

service sipxecs stop
yum clean all
yum update
reboot

It may not be necessary to stop the services but I always feel better doing
so.
