"If" it were me, and it's not... I would delete the siptrunk and recreate it fresh on 4.2.1.
On the calls to the ITSP, it should be reasonable for them to tell you what they don't link about the calls (i.e. asserted identity, etc.), as it is not reasonable to guess this from a user standpoint. Also ensure you send all profiles to the server and restart any items it wants. You need to verify looking at services that all are running and nothing is failed for any reason after an upgrade.
