This thread was very useful to me when upgrading a ISO installed 4.0.4 system 
to 4.2.1... I had to make a couple of changes to get it to work though:

(Question, will this upgrade the box to the latest CenOS 5 version, eg from 5.2 
to 5.5? )

Second, in the [sipxecs-stable] section, change the following line:

        
baseurl=http://sipxecs.sipfoundry.org/pub/sipXecs/LatestStable/CentOS/5/$basearch/RPM

to

        baseurl=http://download.sipfoundry.org/pub/sipXecs/4.2.1/CentOS_5/

(Which version of CentOS are we on? 

[r...@pbx yum.repos.d]# cat /etc/redhat-release
CentOS release 5.2 (sipXecs 4.0.4)

All done!

Except in my case my ITSP (Internode) now times out after about 25 seconds when 
I try and make calls! Boo hoo. Trace shows:

-> invite
<- trying
<- timeout

I'll get on the blower with Internode tomorrow and see what they can see.
