Thanks Tony. I added "<always-relay-media>false</always-relay-media>" to the itsp-account section as defined in the sipxbridge.xsd (which is not on our system btw; from sipx 4.2.1 install cd). Unfortunately this didn't help. I restarted SIP Proxy, SIP Trunking and Media Relay before testing, which are the ones that need to be restarted after making changes under Internet Calling in the web interface.
I also noticed that the xsd file you provided didn't define all the settings in my sipxbridge.xml file, so I'm wondering if there is a difference between the 4.2.1 from the sipx install cd and other versions.
