you can better describe what the problem is i guess...

if you are making settings on the gateway they should be to transform
extensions, don't use a default CID...  you can also force a single caller
id...  i just can't tell from your problem description what it is you
actually want to have happen.

leave name out of this, because even if you do transmit a name, the itsp
will overwrite that and send what they are going to send...
