This usually means the BYE is somehow missed by CDR. The causes could be a
busy network, or an ITSP configuration issue or even a cpu/ram issue
busy-ing out a process at the time the BYE is being sent.

If it is only outbound calls, it makes me want to take a long hard look at
the signalling fron the ITSp or gateway...
