This looks like you didn't portforward the RTP port on the REMOTE-NAT router.
your phone probably is setup for port 5000-5012 or something like that. Creat a portforward from the modem to your phone for all those ports on UDP. That will probably help. (Also disable any Application helpers of ALG 's theat the modem might have.)
