i have seen issues with doing updates on sipx in the past with 4.2 and 4.4 where the MWI light stayed on after the update. Normally going to the box and leaving a VM and then deleteing it triggered it off, but these were lines that had no voicemail enabled, making for a lot of work.
it would be good if you got a snippet of the sla log to see if there is any telling message, same goes for mwi.
