Mike,

Just take a Patton Smartnode gateway (like a 4960), make the SIP side
register to the ITSP and have a PRI going into the old PBX.

The Smartnode has the 'smarts' to be able to login to a SIP trunk provider
like voip.ms.  Then you have a simple single box solution.
