I guess based on my earlier posting, I figured the call flow was obvious but 
it's as follows.

SIP trunk from appia registered as gateway on sipx
User created with DID from appia using this trunk.
SIP phone registered to sipx using this user account.
SIP phone can make outgoing calls no problem.
Using cell phone to call above DID leads to VM if left to ring but ends if call 
is picked up.
