I'm confused at this point because I also have flowroute and voip.ms on that 
same system.
All three are configured as gateways and all work but appia.
Flowroute and voip.ms both allow you to pick the port you want so I have them 
set to 5080.
The only difference I can think of is that appia doesn't allow port 5080 IP 
auth but does allow a user registration over 5080.
I also have a mediant pri gateway being used on the same system.

I don't have any special settings on the appia gateway that I don't have on the 
others.
I do have 'INVITE From ITSP Account' checked as per Tony's suggestion.

All are authenticated as they should be.

I'm not sure what you mean when you say that I have something fundamentally 
wrong with my setup since everything else works?
