I have 8k+ and have definitely lost images not sure how. Also there was that 
TMrelated bug a while ago at OS Upgrade. Hard to tell where they disappeared. 

Had the same thing moving iTunes libraries too (when done through iTunes). 

So near-perfect in te last few years - but not 100% here.
