The last kernel extension is IOUSBFamily, so that would definitely point 
towards a USB port causing an error. If the kernel panics are happening daily, 
try taking it to the people who installed the front ports and get them to 
disconnect & test. 

Have you tried the standard SMC & PRAM resets? Also disconnecting all USB 
peripherals and putting the MacPro to sleep?
