I would say: If the DB is on the same machine, then it is most probably used only for Spacewalk. So restart it. Using box with Spacewalk for something else is not recommended. I would say that include DB. Standalone DB on different machine is different thing.

But of course, best way will be presence of option in /etc/rhn/rhn.conf
database.restart = 1
And if user use DB for something else, he will set it to 0.
