I have only had this happen once, and it was quite some time ago.  
Zapping the PRAM and Rebuilding the Desktop fixed it for me.  It sounds 
like you have tried that already.

If it were me, rather than do a system reinstall, I would (1) Zap the 
PRAM again (2) Rebuild the Desktop again (3) shut down and pull the PRAM 
battery (4) push the CUDA (5) put the PRAM Battery back in (6) restart.
