This works with FireFox, opening a new tab with the form in it that I
can download.
I have tried both with the Real UA and a UA for FireFox 42, so I do not
know what else to tr to get SM to work on this.

Is your problem happening when you first go to
<https://mybenefitwallet.com>? Or does it happen when you try to login?
I have to login and access the tax forms page from the drop-down menu.
Obviously only someone else with an account with BenefitWallet can
attempt do diagnose this further. I took a look at the page info and
source, but could not make much out of it.
