This works with FireFox, opening a new tab with the form in it that I
can download.
I have tried both with the Real UA and a UA for FireFox 42, so I do not
know what else to tr to get SM to work on this.
Could it be that SeaMonkey's popup blocker is preventing the site from
opening a new tab? To check, you could temporarily disable the popup
blocker at Edit > Preferences > Privacy & Security > Popup Windows,
deselect "Block unrequested popup windows". If it works with the popup
blocker disabled, you should be able to reenable it and add an exception
to allow popups for just that one site.
Someone hand that man a cigar! I went back to the page and attempted
again to access the form. I got the bar across the top about a popup
and changed the setting to permit that site to open popups. I tried
again and the form was available for download.
