This works with FireFox, opening a new tab with the form in it that I
can download.

I have tried both with the Real UA and a UA for FireFox 42, so I do not
know what else to tr to get SM to work on this.

Could it be that SeaMonkey's popup blocker is preventing the site from
opening a new tab? To check, you could temporarily disable the popup
blocker at Edit > Preferences > Privacy & Security > Popup Windows,
deselect "Block unrequested popup windows". If it works with the popup
blocker disabled, you should be able to reenable it and add an exception
to allow popups for just that one site.

Or perhaps an add-on such as NoScript or AdBlocker? To check that, use
Help > Restart with Add-ons Disabled. If it works in that mode, you'll
need to figure out which add-on is causing the problem and either add an
exception for that site or permanently disable the add-on.

In the past, I had a similar problem with the USPS site. The problem was creating/printing the forms for international packages. It had something to do with the fact that I did NOT use Adobe Reader but rather Foxit Reader. Could something similar be effecting you?
