Can you not still have them snail mail you a hardcopy?
Here where I live that is still the main method of obtaining forms, the
internet option is new and they are very stupid in that stuff yet.
I'd say hardcopy is pretty darn accessible!! :-)
Oh, and it's a combination of ignorance (lack of knowledge) and stupidity
