You can buy a bare SIM and supply your own phone.  My T-Mobile SIM sits in 
an unlocked Rogers Nokia 6590i and works just fine.  I can put it in any of 
of my other unlocked GSM phones and use it if I prefer.

I got my SIM off eBay pre-activated (the cheapest way to get them; you may 
have to change your phone number if you want it to be local to where you 
live).
