Well, with number portability there's nothing that's absolutely certain any longer. To find out what codes are assigned to a particular entity you can go to http://telcodata.us There you can search by area code/prefix or by company name. If searching by company name to get an accurate search you will sometimes have to use another name such as a name that they formerly used. e.g. in New England you will not find much in the way of T- Mobile but you will find listings for Omnipoint which is what the company was called before VoiceStream and later T-Mobile took them over.
