You show your membership card at the door for entry, and again at purchase. All services and purchases are tied to your membership account.
There are some regulations that prevent certain kinds of business from being "membership" based - so the pharmacy in the U.S. And Japan (don't know about others) are open to "everyone" and have a separate, non-membership, register everyone uses in their section, but you have to use the normal entrance/exit. But for 99% of customers and products purchased, it is a membership store. But it is a store that many many people have membership to (most people I know have one).
