That's not a particularly good example, as Caernarfon is the Welsh name. The English name is Caernarvon. But in this case, nobody, not even the English, uses the English name any more. The English version though, is found in a lot of historical documentation. So including it somewhere is probably appropriate.
I'm not sure, though, that name:en would be the right tag, as that implies that it's a name in current use. Do we have a way of tagging historic or archaic names? If so, then I'd use that for Caernarvon.
