I think part of the problem comes from always working off of lists of perceived most urgent bugs. Bugs get ignored in the first place because they weren't thought especially urgent when first presented, and they continue to seem that way until they get lost and forgotten. These old issues are important to somebody and, as more accumulate, more users get more unhappy. I would suggest that age of the bug be considered when assigning priorities. A bug that has been in bugtrack for three months (for example) could joint the top of the list.
