Actually what seems to happen is when you make a change, the instructions for the whole toolbar you are changing get written to the tbuser.def. So if you move the trash button on the main toolbar one spot to the left, the instructions on where to place every icon in the main toolbar gets written. This adds many lines for one simple change.
