No, IIRC 'refresh' is something for virtual folders.

The .tbb contains the messages, the .tbi is the index file, there
shouldn't be any .tmp files (I have none) the abundance of .tmp files
might be due to your problems. A multitude of .tmp files in the temp
folder disturbs TB's finer functioning, so you might try to delete
them.

It isn't dangerous to delete a .tbi file, TB will create a new one as
soon as you access that folder again. In general I advise to close TB
before starting to delete .tbi files.

Everybody has his pet application, I guess the ABC converter is
Rick's. ;-) There are more third party tools that are able to backup
TB's message base. Mailbag assistant is one of those (no, I don't use
that one either)
My suggestion would be to export all folders to the Unix mailbox
format. TB can do that by itself (select all msgs in a folder Tools->Export)
And the Unix mailbox format can be read by a quite a lot of mail
related applications.

I wouldn't pin myself down to 50. Currently I've got 185 msgs in my
Inbox and I've never had any problems on that point. My advice is that
you don't use the Inbox for permanent storage. If a message is worth
being kept then you'd do best to move it to a different folder. Those
185 of mine are just 180 days worth of unfiltered (and unmoved) mail,
they'll expire in due time.

It isn't TB default, but that functionality is available right now.
Create a folder called 'Main'
In the account properties you'll find something called 'alternative
deletion'
 Account -> Properties -> Mail management -> Deletion -> Alt deletion
Set this to move alternatively deleted messages to your 'Main' folder
Check the option to use alternative deletion for purging.
Set the Inbox to purge on exit, set the max number of messages to zero
and you're done.

Unfortunately this will purge all messages from other folders to
'Main' too, so if you purge more folders you might want to alter the
alt deletion options for those folders. You can set the alternative
deletion options for all folders except for Inbox, Outbox, sent mail
and Trash.
