Lotus Organizer is now in version 6.  It's far more capable than 2.x.
I have version 5, and it not only allows for very extensive recurrence
templates but also allows customization by setting specific dates. The
alarm will go off after the even, but only when Organizer is started.
For something that would work without the program itself being on, you
probably need to use something as complex as Lotus Notes, which has a
Minder feature that would alert you to mail and calendar items even if
Notes is not started.

Zoot is an incredibly rich information processor with PIM capabilities
that probably would also fit the bill, but I use Organizer (ver. 5) as
my PIM, so can't really comment on Zoot's capabilities there.
