I agree, I think the plug-in approach is rather bizarre in some respects, but makes sense in others, regardless, my email client of choice - Vet - doesn't have a plug in, and i suspect, never will. A work around that works reasonable well, is to make use of the filters. If you receive an email with an attachment, use a filter to extract the file(s) to a specific directory, and within the same filter run a virus scan on that directory. I use a batch file to run the virus scanner, and to delete the files after the scan.
