Not sure if this helps, but if I invoke Message Dispatcher and
re-download the message in question, the Word doc attachment shows up.
*AND* the previous version of message that wasn't showing the attachment
now *does* show the attachment.

Ummm... OK...  So it's "bad" - who/what is setting the content-type? I
thought this was MIME stuff set at the server level?  (this is beyond my
expertise, so excuse my ignorance on the subject)
