I'm in pretty much the same situation. I've always been too busy to index my manuals. However, when we switched to Webworks to create the help it became necessary in order for the help files to have indexes, so now I do hammer together a very basic index for each book that is also used as the source for online help. I've gotten better at the FrameMaker index markers, so I can usually put an index together in about 4 to 8 hours, depending on the size of the book.
