(1994) The Art of Indexing. John Wiley & Sons. ISBN: 0471014494 I also have The Chicago Manual of Style (14th ed. -- 15 is at home) and Microsoft Manual of Style for Technical Publications (Version 3.0). They have sections on indexing.
