We're an OEM manufacturer and provide our customers with copies of our document source files so that they can reuse the content in their own documents. We insert index markers for key words and phrases as we write, but just auto-generate the back matter in the reference PDFs we create for initial delivery and don't devote any time to turning it into a context-based index because we aren't able to predict what the final context for any given entry will be once it's been incorporated into our customers' systems and documents.
