Turning on FrameMaker's "Create Named Destinations for All Pgfs" setting 
indeed increases the file size (and and may also reduce response time in 
Acrobat/Reader). But given the choice between larger PDFs and PDFs that 
have "some bad links", I would definitely go with the first.

It is possible to remove the unused destinations in Acrobat, but this 
should be done only if the PDF is a stand-alone one (i.e. no links in other 
PDFs or web pages pointing to specific locations in it).
Another option is the UnBloat function in my FrameMaker-to-Acrobat 
TimeSavers add-on (executed as part of the distilling process).
