For 1U and 2U machines I can manage, but when I had a 5U machine to install, I borrowed a lifter similar to the one Tom describes (with the spreaders) from our facilities folks. It was a bit awkward because of the spreaders, plus the fact that my machine room is the size of half an office with the door at a right angle to the rack, so I had to put the server on sideways, but it was still a lot easier than it would have been by hand.
I should try to see if some of the other departments might want to go in on one of the ones with the counterweight, which would probably be better.
