It's not very difficult to get to the fan in the T41 (you can download
the instructions from the Lenovo support site). I had a T40 with a
noisy fan, and I opened it up, unscrewed the fan, gave it a good
cleaning (lots of dust, etc. in there) and rescrewed it back. That
fixed it up nicely. I didn't have to bother with new thermal grease,
but would have gotten some if it looked like a lack of grease was the
problem.
